FREIGHT: F.O.B. (from shipping location)

INSURANCE: Lessee must furnish a certificate of insurance naming Allied Machinery Corporation the Loss Payee and the Additional Insured for the retail value of the equipment.

RENTAL CONTRACT: A rental contract must be completed for every transaction with all pertinent data furnished prior to the release of the rental equipment.

HOUR LIMITS: Daily - 8 hours. Weekly - 40 hours. Monthly - 160 hours.

REPAIRS, TIRES & MAINTENANCE: All repairs required due to abuse or negligence will be charged to the lessee upon return of rental equipment. Also, any abnormal clean-up will be the responsibility of the lessee.

FUEL: A full tank of fuel is provided with the rental. Return with less than a full tank will incur a refueling charge.

Equipment availability and rental rates may vary from branch to branch.

Add 4.166% G.E. tax and delivery (4.712% on Oahu)
